Print and fill out
this form as completely as you can.
Remove all
miscellaneous items and accessories from your instrument case
and make sure your instrument fits securely inside the case.
Pack your instrument case in a box, at least four to five inches
taller and wider than your case, and fill the surrounding space
with packing materials so the case is surrounded by packing and
is suspended securely inside the box. Include this form in the
box before shipping.
Insure your
instrument and choose two-way shipping from your carrier of
choice. You are responsible for shipping and return
shipping charges.
Contact Bandman to
let us know that you are sending your instrument for repair.
Once we receive the
instrument, Bandman will contact you with a free repair quote before
doing any work.
